I e-filed my 1040 and need to send in my form 8965 as I'm part of a Healthcare Sharing Ministry. At the top of the form it says to attach to form 1040. Does that mean I need to include a copy of my 1040 even though I filed my taxes already or can I just send the 8965 without the 1040? Also, do I send it to the London, KY address? Any help would be greatly appreciated. They have not made this an easy process!

In general, form 8965 will be generated when you answer certain questions in the Health Care Coverage section in installed software or in Affordable Care Act (ACA) (Forms 1095-A, 1095-B, 1095-C, 8962, and 8965) topic if you use H&R block online products which is part of the "Taxes" section of the return. The software will automatically fill it out (including the exemption code) and submit the form for you with your tax return when you e-file.
You can claim a coverage exemption for yourself or another member of your tax household for any month in which the you and other member in your tax household was a member of a health care sharing ministry. Code “D” in Part III, column (c) is used in form 8965 for this type of exemption.
As of September 1, 2016, the coverage for member of health care sharing minister are no longer granted by the Marketplace and you can claim this exemption directly on your tax return. Therefore, please do not send form 8965 to Health Insurance Marketplace- Exemption Processing center in London, KY.
You only need to send the exemption application to the exemption processing center in London, Kentucky when your exemption is required and granted by Marketplace such as hardship exemption, coverage considered unaffordable…ect.

Form 8965 only gets sent to the IRS. It is normally attached to your tax return.
If you used the H&R Block tax software and you indicated that you qualify for the healthcare sharing ministry exemption, which is exemption code "D", then it has generated Form 8965 for you and you don't need to do anything else.
If you did not include an exemption on your tax return then you'll need to wait an amend your tax return to include the exemption once the return has been processed. An amendment can be completed using Form 1040X. If you amend make sure that you send the 1040X along with the 8965 to the IRS in the mail.
Also note that your tax return will not actually be filed until January 29th. The IRS is not accepting any tax returns before that date, so you actually very early.
