I now know that line #1 on the 8889 needs 'Self-only' checked.

I have spent several hours yesterday and more than several hours today going through the software, HR Block community and internet searches to try and find out. I did not look at line #1 on the 8889 while going through the searches.
Again, thank you.
I may make another post for help on amending the 2015 filing.

Maybe this will get you started....
I used the H&R Block desktop software (download or CD to my PC) in 2015.
I have my 2015 tax file open.
Go to the HELP tab along the top left,
Type "AMEND" in the search box,
You'll see "Amended return", Click "GO THERE" button to start the interview.
I forgot to mention that this amending may affect your state income tax amount as well (depends on your state).

About a half hour ago, when I opened the HR Block 2015 Deluxe, it asked me if I wanted to update the software. I updated it and it automatically updated the return and made me eligible for the HSA contribution and removed the exess. The taxes due dropped by 191. I am now working on the amended return - I overpaid 2015 federal taxes by 191.
