HSA excess employer contributions

For the first 3 months of 2017, before going on Medicare, I had an HSA to which I contributed $1,100 and my employer contributed $200 which is not shown on the W-2.  The total on 5498-SA shows a total of $1,300.  That made for an excess contribution of $200.  Form 8889 line 2 asks for contributions that I made and line 9 asks for employer contributions.  H&R Block Deluxe has no way (that I can find) to enter the amount that belongs in line 9.  The interview for HSAs only has an entry for "other contributions".  I'm at a loss for correctly reporting the HSA contributions.
